Health Networks
The Health Networks (HN) will provide a vehicle for joining up local partnership planning, delivery, involvement and participation from local partners and communities. Health Networks will maximise opportunities for 'joined up' working and partnership delivery at the local level.
Health Networks will provide local intelligence, contributing to the Joint Strategic Needs Assessment (JSNA) and Area Action Partnerships (AAP’s). The JSNA has identified areas that require a greater level of investment in services in order to narrow the health inequalities gap in County Durham. Locality arrangements will play an important role in realising this ambition.
Each of the 5 Health Networks (HN) will develop a health improvement plan in line with priorities of the Health and Wellbeing Partnership. Each HN plan will focus on up to 3 main priorities in each area to deliver the outcomes for local residents; the JSNA provides information at locality level on which these 3 priorities may be based.
The geographies are:
- Dales (including Teesdale and Weardale)
- East Durham
- Derwentside
- Sedgefield
- Durham and Chester le Street
