Structure

The Health & Wellbeing Partnership is one of five thematic partnerships aligned to the County Durham Partnership  and consists of representatives from a range of organisations  across County Durham who are focused on reducing health inequalities in the county in order to achieve goals and targets.

The Health & Wellbeing Partnership (H&WBP) framework consists of the H&WBP board, a coordination sub group and five Health Networks (HN).

Health & Wellbeing Partnership Board

The H&WBP Board consists of both core and linked partner organisations and meets six times per year. Core partners work on the health and wellbeing agenda providing high level strategic leadership and coordination and our linked partners work with the health and well being agenda in a way which is specific to a health issue or population group.

Coordination Group

The overarching role for the Co-ordinating Group is to ensure that the H&WBP has the correct processes and procedures in place to achieve the partnership priorities of:

  • Improve life expectancy
  • Reduce health inequalities 
  • Improve mental health and wellbeing of the population

Health Networks

The Health Networks  provide a vehicle for joining up local partnership planning, delivery, involvement and participation from local partners and communities. Health Networks  maximise opportunities for 'joined up' working and partnership delivery at the local level.  The Health Networks are consistent with the current geographies of the Practice Based Commissioning (PBC) clusters and Local Children’s Boards (LCBs).

The geographies are:

Dales including Teesdale and Weardale
East Durham
Derwentside
Sedgefield
Durham and Chester le Street
